The CareAdvisor program was designed to address the issue of uninsured patients with chronic illnesses who were using the Emergency Room at Baptist Medical Center South for their medical home. Such patients are invited to join the CareAdvisor program, which aims to improve their quality of care and quality of life. Participants have access to medical care through a family practice medicine residency program, as well as access to a case manager and a nurse 24-hours per day.

The mobile clinic, which served more than 3,900 children in 2009 and is on track to surpass that number this year, provides well-child examinations, sick-child visits, immunizations, and community safety events.

The Cardiovascular Care Program seeks to provide comprehensive, high-quality care for patients with heart disease, regardless of their ability to pay. This mission is particularly relevant given the high rate of cardiovascular disease in New Jersey, where the hospital is located. Cardiovascular disease is the single largest killer in the state, accounting for 40% of deaths. Furthermore, more than 15% of the state's population is uninsured and unable to access quality health care.

Wellness on Wheels (WOW) is a 42-foot mobile health clinic that provides free screening services to the medically underserved and indigent residents of DeKalb and surrounding counties, with particular emphasis in the Stone Mountain, Clarkston, and Chamblee/Doraville communities of Georgia. These areas are among the most medically underserved, with a high proportion of uninsured patients who have limited access to health care. With a triage room and 2 exam rooms, including digital mammography equipment for providing screening mammograms, the clinic is able to provide a wide array of services, including screenings for hypertension, diabetes, cholesterol, breast cancer, prostate cancer, hearing, and vision. Free child and adult immunizations and ophthalmologic consults are also provided.

 

 

Vaccines for Children Clinic, Geneva, IllinoisBookmark and Share

The Delnor Vaccines for Children Clinic in Geneva, Illinois, was developed with the help of several community partners and medical providers to meet the needs of uninsured, underinsured, and/or low-income children, and to fill a service void created when a similar federally-funded program was being cut due to budget deficits. In its first year, the clinic provided vaccines for 13 children - all free of charge; in its second it served 50 children per clinic.

 

CareDirect Patient Referral Program, Longview, TexasBookmark and Share

The CareDirect Patient Referral Program is designed to reduce the use of the emergency department for primary care services by helping ensure patients have a primary care physician. Upon discharge from the emergency room, patients without a primary care doctor are referred to the CareDirect coordinator, who assists with scheduling a doctor visit. Referrals are made to several practice locations, based on the patients' health and payment type. Since the program's inception in 2009, CareDirect has met with nearly 1,900 patients, resulting in 644 referrals to a primary care physician or specialist.

 

Dr. Miles and Lib Mason Children’s Clinic, Lawrenceville, GeorgiaBookmark and Share

The Dr. Miles and Lib Mason Children's Clinic in Lawrenceville, Georgia, is dedicated to serving the uninsured and underinsured children in Gwinnett County by providing both preventative and sick-child care around the clock. Nearly 26,000 children have been served this year alone, and the clinic is also a state leader in the volume of pediatric vaccines administered, providing 24,500 vaccines to children each year.

The Hocking Valley Community Hospital Food Drive in Logan, Ohio, began 5 years ago with a contest to see which hospital employees could donate the most food items to a local organization that provided assistance to needy families. Held in conjunction with World Food Day, the event yielded more than 3,000 food item donations for families in the rural community. The need for donations has grown, and the annual event has continued; more than 5,000 food items were delivered in 2009.

The InovaCares Clinic for Women and Children, which works in partnership with the Fairfax (Va.) County Health Department, was established to address specific health care access issues for the uninsured and Medicaid population of Northern Virginia. The Clinic for Women offers a wide range of Ob/Gyn services for those in need, and the Clinic for Children provides a secure and affordable medical home for children who are uninsured or on Medicaid. The Clinics provide comprehensive care through direct clinical services, engage in research, and provide graduate medical education for medical students and residents.

The Community Aid, Relief, Education and Support (C.A.R.E.S.) Clinic is an innovative free medical clinic and service learning experience for interprofessional students. In addition to providing much needed health care to the uninsured population in several counties around Charleston, S.C., the clinic, which is fully supported by the Medical University of South Carolina, also provides a service learning experience that has enhanced the preclinical experience of medical, physical therapy, pharmacy, hospital administration, and physician assistant students.

The Mobile Food Pantry is a collaboration between NorthCrest Medical Center, Second Harvest Food Bank of Middle Tennessee, and the Robertson County United Ministry Program to deliver food to agencies to hold a one-day large-scale food distribution event for families in need. Events are held quarterly, with each providing food for up to 500 families. NorthCrest Medical Center hosts the event, and employees volunteer their time to assist with distributing and loading the food.

 

 

Sister & Sister Free Mammogram Program, Oakland County, MichiganBookmark and Share

The Sister & Sister Free Mammogram Program was developed to increase mammogram screening and survival rates among hard-to-reach uninsured and underinsured women in Oakland County, Michigan. Since its inception, more than 1500 women have been screened free of charge. The program also now partners with low-income African American churches to further extend its reach using a faith-based approach.

The Pine Ridge Regional Medical Clinic provides much-needed specialty medical care to the Oglala Sioux people of Pine Ridge Indian Reservation in the remote southwest corner of South Dakota - one of the most medically underserved and poorest communities in the nation. Specialists who provide diabetes care, warfarin management, podiatry care, and cardiac and pulmonary disease care, are available to the residents through the clinic. The clinic also provides public health and wellness fairs where reservation residents can receive free health screenings.

 

Dean & St. Mary’s Neighborhood Asthma Clinic, Madison, WisconsinBookmark and Share

The Dean & St. Mary's Neighborhood Asthma Clinic is a collaboration between Dean Clinic, Dean Health Plan and St. Mary's Hospital to address the needs of medically underserved residents in Madison, Wisconsin's low-income areas. The weekly clinic is run by a nurse practitioner and is staffed by volunteer physicians and other providers from the sponsoring organizations. Patients served are primarily low-income residents who may not otherwise receive medical treatment for their asthma, or who only seek care through urgent care clinics or emergency departments.

The MobileMed/NIH Heart Clinic at Suburban Hospital in Montgomery County, Maryland, is a partnership between the hospital, Mobile Medical Care Inc. (which provides free or low-cost care to anyone in need) and the National Institutes of Health. The purpose is to address the high rate of cardiovascular disease, which is the second leading cause of death in the community. The program works to meet the cardiac specialty care needs of the county's vulnerable uninsured residents. Every week, cardiologists, surgeons, nurses, and administrative and support staff donate their time to deliver high quality and personalized specialty care to these patients.

 

Sobering Center of Tacoma, Tacoma, WashingtonBookmark and Share

The Sobering Center of Tacoma, Washington is a 24-hour supervised facility designed to divert homeless, intoxicated adults from local hospital emergency departments when the individuals do not need medical attention, and to connect them with community resources that can change their lives. In partnership with various organizations, such as the local fire and police departments, the Sobering Center works to improve health outcomes of homeless, alcohol-dependent individuals through health screening, case management services, and by providing a comfortable bed and a nutritious meal.

The Trinity Center for Women was born out of a desire on the part of physicians and midwives of OB/GYN Associates in Columbus, Georgia, to serve the underserved expectant mothers in the community. The team of dedicated clinicians joined forces with St. Francis Hospital, which offered its support in providing superior obstetrical and gynecologic care to women who live in poverty, who have little or no insurance, or who have no access to quality care - regardless of the reason.

ARCHES focuses on six domains or themes, including Access and Advocacy; Research, Evaluation, and Outcomes Measurement; Community Partnerships and Outreach; Health Education, Screening and Prevention Programs; Education of Health Professions Students and Providers; and Service Delivery Systems Innovation. Key initiatives include care for the homeless, breast health education for sheltered women, diabetes self management education, community collaboration addressing obesity, a refugee health center, and hypertension, stroke and prostate cancer education.
