The Aspirus Community Partnership Fund is a collaboration between Aspirus Wausau Hospital and Women's Community, Inc. - a private nonprofit organization that provides specialized services to people in north and central Wisconsin who are affected by domestic violence, sexual assault and unemployment. Together, the organizations have developed a number of programs, such as the Aspirus Women's Care Suite at the hospital's emergency department, a women's shelter adjacent to the hospital, and a community-side education program to prevent sexual and physical abuse of women and children.

The Brooks Clubhouse in Jacksonville is designed to address the unique long-term needs of patients who have suffered traumatic or non-traumatic brain injury. These patients often need support to return to competitive employment and to a life that feels productive.  As "members," participants receive vocational training and rehabilitation in a setting centered around a work-ordered day.

 

Outdoor Adventures, Pomona, CaliforniaBookmark and Share

Outdoor Adventures is a year-round therapeutic recreation program for individuals with various types of disabilities. As part of a medically directed rehabilitation continuum of care, a range of activities, such as water- and snow-skiing and group sports, are offered with the goal of changing participants' perspective. The program serves as a resource as patients leave the clinical setting.

 

Men’s Health Fair and Car Show, Jeffersonville, IndianaBookmark and Share

The Clark Memorial Hospital Men's Health Fair and Car Show is an annual event in Jeffersonville, Indiana to increase awareness among men and their families about the importance of prevention and early detection of disease and chronic conditions. The Health Fair is held in conjunction with a car show to encourage attendance among men, who typically do not seek preventive health screenings.

 

Prescription Delivery Program, Walton, New YorkBookmark and Share

The Prescription Delivery Program is a partnership with a local pharmacy to make it easier for patients in rural upstate New York to access pharmacy services. Residents in the Catskill Mountain region served by the hospital often have difficulty - especially in winter - navigating the mountainous terrain to get to distant pharmacies. The program provides delivery of prescriptions via hospital courier to outlying primary care centers, where patients can more easily pick up their medications.

 

Prostate Cancer Screening Program, Washington, D.C.Bookmark and Share

The Prostate Cancer Screening Program was developed to offer community-based prostate cancer education, screening, patient navigation and treatment to minority men, regardless of their insurance status or ability to pay for treatment. Mass health events, community partnerships and screenings have been used to provide free screening to more than 2,000 men, and education to countless more over the past 4 years.

 

Interpreter Services, Seattle, WashingtonBookmark and Share

Harborview Medical Center's Interpreter Services program is designed to help overcome language barriers and cultural disparities in health care through the services provided to the care team by specially-trained bilingual and bicultural caseworkers. For more than 25 years, Harborview has helped patients with limited English proficiency through Interpreter Services and community outreach.

The Trauma Survivor Reunion program organizes an annual gathering of survivors of trauma, which helps to promote healing and reconnections among participants, and to provide closure for both patients and staff.

 

Cancer Clinic LIFT Program, Florence, South CarolinaBookmark and Share

The McLeod Cancer Clinic in Florence, S.C., was created as a not-for-profit free cancer clinic, and it is the only one of its kind in the state. The clinic includes a radiation oncology program, and more recently, another program was added, entitled Loving Initiative for Transportation (LIFT). LIFT provides transportation to and from the clinic free of charge for those in need of such assistance.

 

 

Memorial Meals Program, Union County, OhioBookmark and Share

The Memorial Meals Program is dedicated to providing quality nutrition to the residents of Union County, Ohio, and surrounding communities to meet 1/3 of the Recommended Dietary Intake for older Americans. Service is provided through three separate service lines (Mobile Meals, Community Meals, and Supplemental Meals) to older adults in need of assistance. Mobile Meals is a hot meal delivery service for those over age 60 years, Community Meals provides meals in a friendly, sociable atmosphere at three different community sites, and Supplemental Meals is a meal delivery service for those under age 60 who need assistance.

 

Care Car Program, Chanute, KansasBookmark and Share

The Care Car Program provides essential transportation services in four counties in Southeast Kansas for elderly and disabled patients in need of outpatient surgery, treatment at the Cancer Center, cardiac rehabilitation, physical therapy, occupational therapy and general medical care. The goal is to provide regional access to hospital services for the area's isolated elderly and to help them sustain a reasonable, healthy quality of life. In 2009, more than 2,850 transports were made.

 

Foundation for Healthy Communities, Concord, New HampshireBookmark and Share

The New Hampshire Health Access Network was established as a response to the increasing number of uninsured and underinsured New Hampshire residents who were not eligible for any public assistance programs and who were unable to afford healthcare. The statewide program, which involves all 26 New Hampshire hospitals, provides a uniform approach for identifying residents in need of assistance and opening up access for patients throughout the state.

 

Emergency Department Consistent Care Program, Olympia, WashingtonBookmark and Share

The Emergency Department Consistent Care Program was developed in conjunction with the CHOICE Regional Health Network - a nonprofit community health collaborative - to reduce inappropriate use of the emergency department, improve the health status of participating patients, and increase the capacity and integration of safety net services for patients. Emergency department physicians and nurses identify candidates in need of coordinated care and refer those patients to the program so they can get that care. Program participants generally have uncontrolled, severe chronic conditions, and more than 80% appear to be drug-seeking. Few have strong primary care provider relationships.

 

A Journey with Joe into Diabetes, Maryville, MissouriBookmark and Share

A Journey with Joe into Diabetes is a novel approach to diabetes prevention that targets elementary grade students across northwest Missouri and beyond with a fun and interesting prevention message. The program has involved live presentations that have been attended by more than 1,500 students and teachers, and it is now being expanded with the creation of a partially animated DVD, which can directly reach thousands more children and their families.

Transylvania Regional Hospital's Rx Assistance Program helps low-income patients with chronic conditions and ongoing medication needs to obtain those medications through free programs offered by pharmaceutical companies.

 

Assertive Community Outreach, Kansas City, MissouriBookmark and Share

Assertive Community Outreach is a program that has helped hundreds of individuals living in shelters or on the streets of Kansas City exit homelessness. Many of these individuals have serious mental illnesses and co-occurring substance abuse disorders, and they have received health care, engaged in on-going mental health treatment, and have secured income and housing through the program, thereby improving their overall quality of life.

 

Senior Services Healthy Living Connections, Tucson, ArizonaBookmark and Share

The Senior Services Healthy Living Connections program supports older adults during and after a hospital stay, as well as year-round through wellness programming aimed at enhancing the well-being of the entire Southern Arizona senior population. This is accomplished through various programs, including the volunteer Seniors Helping Seniors program, a Pet Therapy program, and a Post-Hospital Support Services program.

 

Amish Health & Safety Initiative, Chardon, OhioBookmark and Share

The Amish Health & Safety Initiative was developed to help meet the unique health and lifestyle needs of the more than 12,000 Chardon, Ohio area Amish residents. Through a variety of free health and wellness programs, including a Health & Safety Day, the Initiative provides health and safety education and resources. Educational programming and materials, as well as safety equipment such as bike helmets and car seats are provided.

 

Outpatient Care Management Program, Galveston, TexasBookmark and Share

The Outpatient Care Management Program in Galveston, Texas, is an integrated care and disease management model that focuses on high-risk uninsured, financially eligible UTMB patients with chronic disease, such as congestive heart failure, diabetes, coronary artery disease, renal disease, asthma, and others. The goals of the program are to improve disease management, reduce complications, provide patients with a primary care provider, and decrease utilization of acute care services related to chronic disease.

 

Mary's Gift, Decatur, TexasBookmark and Share

The Mary's Gift program provides free diagnostic and screening mammograms to low-income, uninsured women in Wise County, Texas. Founded in 2005 by a breast cancer survivor, Mary's Gift is now administered by the Wise Regional Health Foundation, which organizes fund-raising events throughout the year to promote and fund the program.
