Thanks to the efforts of Bon Secours St. Francis Health System's Healthy Community Initiative, which is working in partnership with community residents, the neighborhood is making its way back after years of deterioration.

 

Operation Outreach, Memphis, TennesseeBookmark and Share

Baptist Operation Outreach is a mobile health care unit designed to address the primary care needs of the homeless in Memphis, Tennessee. The program provides acute and primary care, disease prevention information, screenings, and guidance for those without permanent housing. Health and development assessment and immunizations are also provided for children.

The pastor at the Cape May, New Jersey church was stunned one week, when "Helen" rose from the pew and, without her walker, made her way to the serving table to receive Holy Communion.

Dixie Regional's Physician Referral Program was created to connect patients with healthcare providers. The program has been especially helpful for Medicare and Medicaid patients, who make up 64% of Dixie Regional's payer mix, and who often have difficulty finding physicians willing to accept Medicare and Medicaid patients.

The Community Wellness Program, a prevention-focused program that works to provide community members with the knowledge, understanding, support and follow-up they need to become proactive in their health  and to successfully manage or reduce their chances of obtaining a preventable chronic condition.

 

Community Services, Danville, KentuckyBookmark and Share

Ephraim McDowell Health Community Services is an innovative, outcomes-oriented program for addressing unmet healthcare needs in the Danville, Kentucky area - a predominantly rural region. Based on a needs assessment, the program's focus is on improving access to health care for underinsured and uninsured low-income residents; providing health screening and early detection of heart disease, cancer and obesity; providing education on disease prevention and well-being to help individuals develop achievable measurable health goals; and mentoring health-career students who will reduce the region's shortage of health-care professionals.

 

Haiti Relief Trip, Dallas, TexasBookmark and Share

A team of staff members from the full-service acute care hospital scrambled to collect enough medical equipment and supplies to fill three chartered planes. Those able to travel to the region did so, while others remained behind to help coordinate the efforts. Those who made the trip treated hundreds of injured children and adults and saved hundreds of lives. In order to provide the rapid response necessary under the circumstances, the hospital absorbed the majority of the cost of the mission. 

 

Gibson General Healthcare Foundation, Trenton, TennesseeBookmark and Share

The Foundation, which is an affiliate of West Tennessee Healthcare, has worked tirelessly to seek out and serve those programs and individuals within its rural community most in need of assistance. The 25 volunteers who comprise the board used $50,000 in seed money to launch projects ranging from construction of a heliport for an air ambulance service, to supplying individuals with financial assistance for medication and utility bills. The Foundation sustains its charitable work through fundraisers, and also has established scholarship funds, provided exercise equipment and other support for a local YMCA, donated golf carts to disabled farmers so they could continue to tend their crops and livestock, sponsored a riding academy for disabled children, and provided tuition for adult cancer patients and survivors to attend a special camp.

 

Safe Ride, Allentown, PennsylvaniaBookmark and Share

Lehigh Valley Hospital's Safe Ride program works with pregnant women and expectant fathers to promote the proper use of infant car seats. Through collaboration with a number of community organizations and businesses, Safe Ride provides car seats and education on their proper use to thousands of individuals. In 2010, more than 6,000 have been reached through the program, and since 2007, more than 11,000 demonstrations and baby seat checks have been conducted. 

 

Thanksgiving Meal in a Bag Program, Monroeville, AlabamaBookmark and Share

The Thanksgiving Meal in a bag program was initiated in 2008 by Monroe County Hospital in Monroeville, Alabama, as a way to provide a Thanksgiving meal to the sick and needy in the community. In the first year, 150 families were provided with a meal, and the goal this year is to provide 200 meals. The program is made possible by generous donations from hospital employees and community members.

 

 

Mission H2O: Maji for Kenya, Charleston, South CarolinaBookmark and Share

Mission H2O: Maji for Kenya was a program designed to help provide safe, clean water to an African community. Roper St. Francis Healthcare worked with the non-profit Water Missions International and local employees, doctors and volunteers to deliver a Living Water Treatment system to the Plateau Mission Hospital in Kenya, the surrounding community, and its primary school, serving an area of more than 3,000 people. As a result, the Plateau Community now receives an average of 5,000 liters a day of clean water for drinking, bathing, and cleaning from a sustainable source - and the hospital has reported a dramatic decrease in the rate of waterborne illnesses.

 

Scripps Medical Response Team, San Diego, CaliforniaBookmark and Share

The San Diego-based Medical Response Team, created in the wake of the 9/11 terrorist attacks in 2001, provided relief staff for a temporary clinic in Houston to help victims of Hurricane Katrina, which hit New Orleans and other areas of the Gulf Coast in 2005. In the first 24 hours, and over several weeks, 24 physicians and 36 nurses treated more than 5,100 patients at various clinics. Eight administrative support staff also joined the effort. Since Hurricane Katrina, the Medical Response Team has continued to respond in times of emergency, including during the 2007 wild fires in San Diego, and in the aftermath of the devastating earthquake in Haiti.

 

The SHARE Initiative, Atlanta, Georgia Bookmark and Share

The SHARE Initiative is a collaboration between Shepherd Center, TRICARE and Humana Military Healthcare Services to provide a continuum of rehabilitation care for soldiers who have sustained a spinal cord injury or traumatic brain injury while on active duty in Iraq or Afghanistan. The program provides assistance, support, and education to these soldiers and their families as they return to their home and community so they can achieve the highest levels of independence and quality of life

 

Community Initiatives, Baltimore, MarylandBookmark and Share

Community Initiatives is a unique spectrum of health and social services offered to the residents of southern Park heights, an impoverished, predominantly African American community where Sinai Hospital is located. The primary focus is on providing services to low-income minority women and their children, whose health and well-being are threatened by generational poverty and many associated social, economic, and health disparities. Services range from HIV support, home visits for high-risk mothers and infants, male youth outreach and violence prevention, fatherhood support, substance abuse counseling and support, chronic disease management and family violence crisis intervention and support.

 

The Wallace Road Clinic, Nashville, TennesseeBookmark and Share

The Wallace Road Clinic in Nashville, Tennessee, along with United Neighborhood Health Services, seeks to serve Latino patients with non-emergent healthcare needs in a clinic setting, rather than in the emergency department, and thereby providing those patients with a primary care home. So far in 2010, 565 patients have been referred to the clinic by the Southern Hills emergency department, with a remarkable percentage of patients (65%) keeping their appointments.

 

Missionaries to Iowa, Caroll, IowaBookmark and Share

The "Missionaries to Iowa" program partnered with the local religious community to train Catholic nuns from Africa and India to become certified nursing assistants, thereby alleviating an acute shortage of CNAs and allowing St. Anthony to construct an Alzheimer's unit that currently benefits 18 residents and their families within the seven-county hospital service area.  The program has the added benefit of allowing the participating sisters to send earnings back to their home countries to provide their own family and community members with basic living necessities.

 

Project H.E.A.L., St. Cloud, MinnesotaBookmark and Share

Project H.E.A.L. (Health, Education, Access, Link) is an outreach program of Mid-Minnesota Family Medicine Center, St. Cloud Hospital and several other community partners. The program offers free health screenings and basic care at six central Minnesota locations eight times per month for uninsured or underinsured individuals. The goal of Project H.E.A.L. is to overcome barriers to health care for the underserved population in the area, and it does so with the help of numerous dedicated volunteers.

St. Luke's Episcopal Health Charities is a unique public charity anchored in public health principles and focused on reducing health disparities among the most underserved and vulnerable populations throughout a 57-county area in southeast Texas. The research-based, grant-making, non-operating charity works to take quality healthcare out into the community by providing funding to nonprofit community health services programs.

 

Medical Transportation Services, Streator, IllinoisBookmark and Share

The Medical Transportation Service was established to provide free transportation to and from medical appointments for Streator, Illinois area residents in need of such assistance - including those who are wheelchair bound. Service recipients are primarily Medicare and Medicaid insured patients, and the program helps them maintain independence while ensuring they get the healthcare they need.

 

T3 (Triage Transport and Treat), Sacramento, CaliforniaBookmark and Share

T3 works with various community partners to provide residents in the Sacramento area, who chronically use the emergency room for non-emergency care, with a means of obtaining the health care services they need through a primary care provider. The vast majority of program participants are homeless, and have mental health and/or substance abuse issues.

 

Wabash Valley Rural Telehealth Network Program, Terre Haute, IndianaBookmark and Share

Union Hospital's Wabash County Rural Telehealth Network Program provides internet-based and live video telemedicine consultations to bridge the gap for the provisions of much-needed specialty services to rural providers. Services range from asthma and diabetes education to tele-mental health, neurology, tele-cardiology, and pediatric burn follow-up care and stroke treatment.

The University of Tennessee Global Health Initiative is a program for university internal medicine residents who wish to gain firsthand experience in healthcare administration in a developing nation. The program, which is an extension of a medical missions project, provides residents with an opportunity to learn the "art of medicine" during a 1-month elective period at the Maseno Mission Hospital in Kenya. Participants become fully involved with the local community, providing healthcare and working with service projects that focus on the needs of orphans and HIV patients.

The Clinic is providing an increasing amount of care thanks to a growing response to an information and education campaign.
